Form DD 2793 consists of four sections or parts: the information provided in DoD 1100.21 implements policies and procedures for the acceptance and use of voluntary services in Department of Defense programs, as approved by 10 U.S.C. 1588. The instructions described below in the form DD 2793 come from the manual published on March 11, 2002. Each branch requires a separate set of documents, but there are several general forms that most volunteers must submit when working. No volunteer has the right to start working until they can do so officially. It is mandatory for all applicants to undergo a background check, provide a copy of their Form DD 2793 and, in some cases, complete Form DA 5018-R, the ADAPCP Client Consent Form for disclosure of Processing Information (LRA). This is a form that collects all the information that can reveal whether a person has ever struggled with excessive alcohol or drug use and has undergone rehabilitation for these addictions. Form DD 2793, Volunteer Agreement for Appropriate Fund Activities or Inappropriate Fund Instruments, issued by the Department of Defense (DoD) on March 1, 2018. This form is mandatory for all volunteers who wish to participate in appropriate or unused fundraising activities. An up-to-date, fillable version of the DD 2793 volunteer form can be downloaded below. The fact that voluntary services are accepted is confirmed on DD 2793 before the person can provide voluntary services for the DoD. A copy of the completed and signed agreement is sent to the volunteer before the start of his work.
A complete overview of the form can be found in the user manual published by DoD 1100.21. . . .
